There is really no way of adding a comment box to the right of each question. It would have to be below each question. We initially looked at adding a comment box for each question, but the survey became more cumbersome complete and manage. So we decided to use the single comment box as a catchall....

I’ve found it always useful to provide an optional comment block with each question in the survey for the participants to provide additional information, rationale, or explanation for their responses.
