Hi all! My name is Brittany, I am a senior at Clemson University finishing my senior design project. I need to do a review of a device and I am looking for anyone that can help me. I am reviewing the Flex-foot Cheetah(R). If you or anyone you knows has used the Flex-Foot Cheetah(R) if you could please answer a few questions for me and give me just a little overview of your experience with running prostheses I would really appreciate it!

1. How many running prostheses have you worn?
2. How long have you been wearing the Flex-Foot Cheetah(R)?
3. How would you rate the comfort of the device on a scale of 1-5 (5 being the most comfortable)?
4. How would you rate the device performance on a scale of 1-5 (5 being the best)?
5. Compared to other devices, how does the Flex-Foot Cheetah(R) perform? A lot Worse Worse About the Same Better Much Better
6. How easy is it to put the device on on a scale of 1-5 (1 being the easiest)?
7. What activities do you perform while wearing the device?
8. Are you satisfied with your device?
9. On a scale of 1-10, what is your overall rating of the Flex-Foot Cheetah(R) (10 being the best)?
10. Would you recommend this device to a fellow amputee?
