I'm a family physician and long distance runner, age 56.

I had meniscus "repair" (likely just debridement) 28 years ago.

I have had intermittent knee pain for the last decade.

I adopted a midfoot strike running form about 5 years ago, which seemed to reduce frequency of flare-ups of knee pain, which is always in medial joint space.

I usually run in 4 mm drop shoes but recently tried 0 drop shoes .. marketed as "more natural."

Knee pain much worse in 0 drop. Back to 4 mm ..pain gone. I can run 10 miles or more without discomfort.

Idea: In folks s/p medial meniscus surgery, when less meniscus is present, loads on medial knee are reduced in runners with midfoot strike by raising the heel, causing the knee and ankle to be slightly flexed on foot strike, perhaps moving maximal force more anterior, where (perhaps) meniscus remains or is more intact.

Hmmm.
