New features
- Added test cases for all currently supported features
- Re-introduced stenosis model based on doi.org/10.1109/TBME.2003.812201

Bugs fixed
- PRESSURE outlet boundary condition produced incorrect results
- PRESSURE outlet boundary condition implicitly converted a value in the input file from mmHg to dyn/cm^2
- OLUFSEN material model produced incorrect results
- tabs caused input file to be read incorrectly
- simulations randomly crashed after time step 1000
